have completed the new patient forms for the Alexandria Chiropractic Center.
You should have printed 4 forms. If you do not have 4 pieces of paper, click
the "BACK" button on your browser to get back to the info you already
typed in and click the "PRINT" button on your browser. Paper clip
or staple the forms together to prevent separation. Take the forms into Dr.
Brown's receptionist, be sure to schedule an appointment first. By filling out
the online forms, you have saved yourself a lot of time in the Dr.'s office.
